Follow-up Consultation  [1/2 hour with Dr. McCreadie $100]

Fees for therapies [nutraceuticals, homeopathic remedies, etc.] are due at time of service.

Medical Insurance

Dr. McCreadie participates with BCBS [Blue Cross Blue Shield]

If BCBS is the insurance carrier for your child, please call BCBS and ask if Dr. Susan McCreadie participates with your plan of BCBS.  If so, your office visit will be billed on your behalf to BCBS.

Except for BCBS, the fee for services rendered is due the same day of service in the form of cash, check, or charge. 

After payment, Dr. McCreadie will provide the parent with an invoice to forward to their child's insurance carrier for reimbursement.  Please contact your child's insurance carrier for their current procedure on how to obtain reimbursement for your child's office visit.
